There is a dangerous pattern in many healthcare settings, especially in large organisations in Nigeria: the reluctance to involve experienced specialists early. Too often, systems wait until a patient is already critically ill, moribund, or the situation has become irreversible before calling for advanced care. Only then do the urgent calls fly in every direction, with everyone suddenly demanding miracles while looking for someone to blame.

This is not safe medicine.

There is a clear reason why some doctors are consultants and others are medical officers. The roles are different for a reason. When a medical officer begins to operate beyond their training and trying combinations and permutations that belong to specialist practice and the patient is placed at unnecessary risk. The human body is fragile. It does not forgive preventable errors, and not every complication can be reversed once it has occurred.

Knowing your limits is not a sign of weakness. It is a fundamental principle of responsible medical practice. Advising organisations to escalate to specialist care early is not an admission of failure and it is the mark of a professional who prioritises the patient’s safety over convenience, ego, or institutional delay.

Safe medicine requires the right doctor at the right time, not the hope that someone will somehow fix what should never have been allowed to deteriorate.
