🔹 Stage I: 62% lower 1-yr mortality, median OS gain of 19 months

SATEEN Trial: ADC Sequencing Lesson
✅ Simple Breakdown:
• Who was studied?
Heavily pretreated HER2+ metastatic breast cancer patients who had already progressed on T-DXd (Enhertu). Median 5 prior lines, 33% had brain metastases.
• What was tested?
Switch from T-DXd to Sacituzumab Govitecan (Trodelvy) + trastuzumab.
→ Changed the target (HER2 → Trop-2)
→ But kept the same payload (TOPO1 inhibitor)
• What were the results?
ORR: 3.7%
mPFS: 2.3 months
→ Limited clinical activity. Trial stopped early.
• Key Takeaway
Changing the antibody target alone gives limited benefit if the payload stays the same.
To overcome resistance after T-DXd, future strategies should focus on changing the payload.
This is an important lesson for ADC sequencing in HER2+ breast cancer.
