Unusual presentation of MPO and pANCA-vasculitis, this week.
Typical presentation is a pauci-immune necrotizing & crescentic glomerulonephritis.
However, none of 20+ glomeruli in biopsy sample showed crescents.
But= Massive necrotizing arteritis in one core (got lucky), plus granulomatous interstitial nephritis.
Diagnosis: Necrotizing arteritis, likely associated with pANCA/MPO vasculitis.
The findings are unusual- more likely to see this lesion in PR3/cANCA vasculitis.
History: 70 year-old man with rapidly progressive renal failure. Serum creatinine 5.5. Positive MPO and p-ANCA titers.
