Some amazing team collaborations have highlighted at #AFHTO2020 Teams have partnered up to set up isolation centre and to help support COVID-19 patients in remote communities with vulnerable population #primarycare
To support mental health and addiction issues, staff provided check-in calls to monitor patients in isolation. Social workers visited homeless shelters to provide in-person care. The team stepped in to provide support to new moms in person or virtually. #AFHTO2020 #mentalhealth
Our teams stepped up. They provided injections in parking lots; provided home visits and backyard visits; and deployed to support assessment centres, hospitals, and other areas of the healthcare system.
In-person care was there for those who needed it.

The pandemic has exposed the social determinants of health and the gaps and cracks in our system in providing care to the most vulnerable. Teams at #AFHTO2020 have shown true teamwork, assigning team members to make dozens, even hundreds of check-in and wellness calls.
There was a massive transition to virtual care as providers pivoted to help reduce the spread of COVID-19 – all without funding, without change management support, and, for many, without a blueprint of what needed to be done. And yet, the #primarycare teams delivered. #AFHTO2020
