The #ComHIP community hypertension program is a partnership between @fhi360 #GhanaHealthService @NovartisFDN that saw an avg reduction of 12mmHg SBP/ 7mmHg DBP among followed up cohort over 1 yr! Achieved through #taskshifting, patient empowerment & use of #ICT4Health #ICUH2018
