Limitations: Stabilization was required within a short period, potentially excluding older, frailer patients, and baseline sacubatril-valsartan use was low. Also the use of a win-ratio analysis design makes this trial hard to directly compare to previous SGLT2i trials. #cvcsccmjc
Strengths: #EMPULSE was performed on both de novo (new) HF patients and previously treated HF patients in exacerbation as well as both preserved and reduced EF. #cvcsccmjc

Safety analysis: Adverse events leading to discontinuation were 8.5% empagliflozin vs 12.9% placebo. No significant differences in adverse events, including ketoacidosis, UTI, volume depletion, acute renal failure, or symptomatic hypotension were noted. #cvcsccmjc
Secondary outcomes: Patients in the empagliflozin group had a greater absolute change in KCCQ-TSS and a greater reduction in NT-proBNP. No significant difference in the incidence of CV death or proportion of patients that had a 10-point increase in KCCQ-TSS. #cvcsccmjc
Primary composite outcome: empagliflozin was superior in 53.9% of comparisons vs 39.7% for placebo producing a win ratio of 1.36 (95% CI: 1.09-1.68, p = 0.0054) in favor of empagliflozin (See Fig. 2). #cvcsccmjc
Components of primary outcome (empagliflozin vs placebo): 11 (4.2%) vs 22 (8.3%) died; 28 (12.6%) vs 39 (14.7%) had a HFE; increase in KCCQ-TSS from baseline to 90 days was 36.2 vs 31.7. #cvcsccmjc
Baseline characteristics were well-balanced between the two treatment groups. #cvcsccmjc
Relevant secondary outcomes include incidence of CV death or HFE, KCCQ-TSS improvement of ≥ 10 points and absolute change, and reduction in NT-proBNP all at 90 days. #cvcsccmjc
