#5 “Dont mistake a label for a diagnosis” - an admonishment to be wary of both #chartlore & diagnostic drecks such as neuritis #chronicfatigue #neurasthenia and perhaps “long” forms of diseases

Opinion: If you write nonischemic #cardiomyopathy in the chart, also write the data this is based on + year (cath 2018; stress 2017, etc) next to it, otherwise you are creating substrate for #chartlore. Pearl: press for 1ary data behind a dx!! Especially if just a phenotype 💜

‘#ChartLore New to me (& I’m a #ProfessionalPt) Incorrectly documented reason for Pt being on particular Med. HT @TxPharmD 2 Thoughts: 1. Review/⬇️/Clean up any Rx errors/confusion @ AnnualPhysical. 2. Bring @myopennotes to your Hospital; #PartnerWithPatients & we’ll fix errors!

@LooseyJackie Our system is totally at risk for errors, too. Can’t tell you how many patients I’ve seen prescribed medications for “chart lore” - incorrectly documented health conditions. It can be difficult to sort through this!

👉🏽 Well, lookie what we have here! Be a skeptic and do your own vessel mapping when referred pts with “no upper arm options” and multiple previous AV access surgeries. @ASDINNews @RenalFellowNtwk #BIGcephalic #ChartLore

Today’s #inpatientpearl - when someone tells you a patient “is somnolent all the time but that is their baseline” that should always raise ur eyebrows & warrant investigation. #chartlore #anchoringbias #treatpatientslikefamily #TipsForNewDocs
