The #CodeICH train keeps a rollin'
This small pilot project from a single ED found faster evaluation and treatment after implementation of a Code-ICH protocol. Antihypertensive Rx given in <30 minutes of arrival in 92% of patients!

More evidence that door-to-needle times for emergency reversal of AC for ICH lag far behind DTN times for IV lytic therapy for AIS (median 49 vs 28 mins).
Lots of room for improvement. The #CodeICH train keeps rolling, and it can't be stoppoed.

In this observational meta-analysis of >400 patients with intracranial hemorrhage on AC, involvement of an emergency medicine pharmacist was associated with significantly reduced door-to-needle times for the 1st reversal agent. benchmark 75 mins (still too slow), but with a pharmacist at the bedside: 20 mins shorter.

The #CodeICH train keeps rolling!
This multicenter study from Spain found that sicker patients present earlier…
That achieving care bundle goals (BP control mostly) faster was associated with better outcomes
And that the benefit was magnified among the sickest patients presenting earliest

The #CodeICH train keeps rolling.
There is accumulating evidence that in many centers it can take 3 HOURS to give emergency reversal agents to pts w/ intracranial bleeding on DOACs.
In this VA system study mean DTN was 200 minutes. There was a 0.5% increase in mortality with each minute of delay in administration.

😳?!
There was a 0.5% increase in mortality for each MINUTE of delay. Choice of agent (andexxa vs 4F-PCC) didn't matter.

Status Epilepticus Protocols vary a lot from center to center. In this analysis of protocols from >200 centers, 1/3 did not specify optimal door-to-needle times, and 15% specified doses that are under what is recommended.
