@RRowanOlive My experience of dialog+ is you tick 5 boxes for stuff you need help with and thenโฆ. Nothing, business as usual (which addresses none).
It was a well intentioned thing by those who created and piloted it but as per, not used in the intended way, ended up as another tickbox. +

@dredbeveridge @nuwandiss @psychsleepy @DoctorAngry There is a danger in the #CommunityMHFramework that trusts use #TheEndOfCPA to pare down their offer, decline to co-ordinate patients, or try to eliminate Standard Care as waste.
In others there is a welcome SMI focus & patient involvement to preserve or enhance these features.
The #CommunityMHFramework needs to learn this. It's not possible to have one assessment at the point of entry which covers all the things that later steps of care might need.

The first diagram here came from the Greater Manchester Mental Health and Wellbeing Strategic Vision of April '21 & is being revised.
My view, is that CMHTs need to map to PCNs (GPs) directly & Living Well should remain an enhanced Primary Care offer, not a modified replacement.

@nuwandiss @Madnotbad60 My fear is also that things like #CommunityMHFramework are limited by the boundaries of what is thought currently possible or part of the envelope. More holistic change should often be broader than the elements on the table.

One good interpretation of this might be an increase in band 5 practitioners to provide all patients a key worker & free up band 6s to have lower case loads & complexity/contribute more focussed pieces of work in line with their expertise, & even more so for the band 7 seniors.
