#Women with #spondyloarthritis (SpA) show markedly shorter persistence on first and second #biologic #DMARDs (bDMARDs) despite similar initiation timing, as demonstrated by real-world data from Spanish REGISPONSER/REGISPON-3 registry:
◦ Study population: 411 SpA patients (271 males, 140 females) followed for 17 years (REGISPONSER → REGISPON-3).
◦ bDMARD initiation: No significant sex difference in time to first bDMARD (from symptoms or diagnosis).
◦ Males showed significantly longer persistence (retention) on bDMARD therapy compared to females:
- First bDMARD: males 88 months vs females 48 months (p = 0.004)
- Second bDMARD: males 260 months vs females 133 months (p = 0.0007)
◦ Multivariable analyses (Cox proportional hazards models) revealed sex-specific predictors of bDMARD initiation.
◦ The main reason for discontinuation was inefficacy in both sexes, but adverse events were reported more frequently as a reason for stopping treatment in females.
These findings indicate that while the timing of starting bDMARDs is similar between males and females with SpA, women demonstrate significantly shorter persistence on biological treatments over the long-term follow-up.

The updated #EULAR recommendations for the management of #rheumatoid #arthritis (RA) with synthetic and biologic #DMARDs have just been published in Annals of Rheumatic Diseases.
These 2025 updated EULAR recommendations are the most concise version yet, further simplifying treatment. They incorporate the latest evidence on efficacy and safety of conventional synthetic, biologic, and targeted synthetic DMARDs, and are summarized in this graphic algorithm.
Key elements unchanged:
◦ Start with methotrexate + short-term glucocorticoids
◦ If this fails, switch to a biologic/ targeted synthetic DMARD
◦ Target remission or low disease activity at every stage
◦ Aim to reach the target within 6 months
Main updates involve fine-tuning:
◦ Initiation of and switching between biologic/ targeted synthetic DMARDs

The first meta-analysis comparing efficacy and safety profiles of #JAK inhibitor (JAKi) versus commonly adopted #biologic #DMARDs for treating children with nonsystemic forms of juvenile idiopathic #arthritis, including newly available secukinumab, did not reveal any statistically significant difference among these advanced treatment options in terms of both efficacy and safety outcomes. Seronegative and seropositive #rheumatoid #arthritis (RA) are probably different diseases and have different patterns of response to #biological #DMARDs (bDMARDs). In a recently published Italian study that assessed the real-world effectiveness of bDMARDs, seropositive RA patients exhibited similar 10-year survival rates for #abatacept, anti-#TNFα drugs, and #tocilizumab, whereas seronegative RA demonstrated a notably different pattern of bDMARDs survival: tocilizumab showed the highest survival rate (73.9%), followed by anti-TNFα (50.0%) and lastly abatacept (37.5%), with a statistically significant difference between tocilizumab and abatacept.
