When we launched the Service in January, we said we would: provide equal access to our care, support people earlier in their journey, enable them to live and die well and avoid admissions to hospital. We have delivered 👏
3 months into our #FrailtyService the headline is clear: it's needed and it works. The next phase involves expanding our #CompassionateNeighbours but to do that, we need more volunteers.

Meet Fiona 💙 She's studying physiotherapy at @UniofHerts and has supported our #FrailtyService launch during her placement with us.
She's been an integral part of our Rehab & Wellbeing team. Here she talks about the need for change in the way we care for people with frailty.
After months of planning, today we launched our #FrailtyService. Our new Link Nurse, Jenny, has already visited the local A&E to identify people suitable for our care. Two patients have been referred.
We can't wait to share further how we are supporting more people, sooner. 💙
