EXPERIENCERS: ENDURING CHANGE OCCURS WHEN AFFECTED COMMUNITIES BECOME PARTNERS IN DESIGNING THE SYSTEMS THAT SERVE THEM. Here is what I can share from what I've learning working in the mental health field. For much of my career, I worked in homelessness, behavioral health, and systems change. One lesson became unmistakably clear: lasting change rarely begins at the top. Government can create policy, allocate funding, coordinate agencies, and establish accountability, but it cannot manufacture trust or community ownership. At the federal level we had an Interagency Council on Homelessness. This guided all grant funding, following local/state/federal laws, resource management, and client/patient care. Our continuum of care in Seattle was known as the "supportive housing network." This was a shorthand for housing with care attached. When we built regional responses to homelessness, we didn't solve the problem simply by creating an Interagency Council on Homelessness. We developed a national strategy, secured sustained federal and local funding, coordinated housing and behavioral health systems, empowered local providers, and relied on people embedded within communities to carry the work forward. Even then, our goal was often not to "solve" homelessness overnight, but to build a system capable of responding compassionately, consistently, and collaboratively. Some of the most transformative work I witnessed did not occur in government offices. It happened on the streets. It happened through outreach workers who knew people's names. It happened in programs like the Plymouth House of Healing (Plymouth House), where Chaplain Craig Rennebohm's companionship model demonstrated that healing often begins not with diagnosis or institutional authority, but with presence, relationship, and the willingness to walk alongside another human being. That experience has shaped how I think about disclosure. Today, much of the conversation appears to be moving in the opposite direction. Rather than growing from communities of experiencers, peer-support networks, clinicians, spiritual caregivers, researchers, and families, the conversation is increasingly mediated by influencers, media personalities, and centralized institutions. Public attention gravitates toward a handful of recognizable voices, while the communities that carried this subject through decades of stigma often remain on the margins. This is not simply a question of personalities. It is a question of governance. A citizen movement matures when the people most affected help shape its priorities. That means building durable institutions—not just audiences. It means developing independent nonprofits, civil rights organizations, advocacy groups, professional associations, ethical standards, research collaboratives, and local support networks that can represent experiencers rather than merely speak about them. Preparedness, in that sense, cannot be reduced to a government communications strategy or a federal task force. Those efforts may have an important role, but they cannot substitute for community leadership. The history of public health, behavioral health, disability rights, HIV/AIDS advocacy, domestic violence reform, and homelessness all point to the same lesson: ENDURING CHANGE OCCURS WHEN AFFECTED COMMUNITIES BECOME PARTNERS IN DESIGNING THE SYSTEMS THAT SERVE THEM. If disclosure continues to unfold, I believe it will require the same kind of infrastructure. Not only scientists and policymakers, but peer specialists, victim advocates, faith leaders, Indigenous knowledge keepers, mental health professionals, educators, families, and experiencers themselves must have a seat at the table. In my own work, I've come to believe that disclosure is not merely about revealing information. It is about building communities capable of integrating profound experiences without fear, stigma, or exploitation. That kind of transformation cannot be manufactured from above. It must be cultivated from within.
