The #GazaPier operation contained several flaws.
1️⃣No boots on the ground. This led to #2.
2️⃣The operation involved the movement of local trucks from the beach to the ships offshore.
It was on the floating pier and ships that cargo was loaded onto local trucks. It was during one of these operations that the accident supposedly occurred.
3️⃣ Typically in JLOTS, the cargo is loaded on military equipment and then transferred to civilian or local trucks in a site ashore.
This is not saying that this is the ultimate cause for the accident, but adding additional cargo handling and movement either on the floating pier or onboard vessels, overall adds an additional level of risk.

The @DOTMARAD MV Cape Trinity has been in the Mediterranean for three months, loaded with relief supplies and unable to offload as they intended to use the #GazaPier. With the demise of pier, they went to the plan they should have used in the first place and offloaded in a port.
The M/V Cape Trinity (T-AKR-9711), a Roll-On/Roll-Off Ship with the Ready Reserve Force of the United States Department of Transportation's Maritime Administration (MARAD), arrived yesterday within the Port of Ashdod in Southern Israel filled with Humanitarian Aid including Food, Water, and Medical Supplies for the Gaza Strip.
