At the #ESCCongress we presented the first data from Poland and CEE on the role of #remnants on #CVD #risk and #outcomes based on the data from the #LIPIDOGRAM cohort #studies (almost 50,000 primary care patients).
We showed that:
👉We showed that 30% of patients in Poland have elevated remnant-C (>30 mg/dl/0.77 mmol/l), and in those with #metabolicsyndrome it might be even 56%!
👉 Elevated remnant-C (the highest quartile) did not increase the risk of all-cause #mortality in 5-year #followup, but were significantly associated with reduced mortality in 15.3-year follow-up.
👉Based on the FMAD and cluster analysis we showed that low remnant-C levels in patients with #comorbidities and more advanced #age may not automatically be indicative of a better #prognosis.

New analysis from the #LIPIDOGRAM & #LIPIDOGEN2015 studies. Using the hierarchical cluster analysis we identified the #phenotypes that have the highest #mortality risk in #primarycare settings.
In primary care, the practice of providing appropriate #prevention strategies to high-risk patients is increasingly recognized as an important #strategy for improving patient care and outcomes, to help optimize the allocation of medical resources and more focus on #patients at higher risk of adverse health #events.
However, challenges still remain in implementing #riskstratification in primary care. Our conclusion that primary care physicians should focus on #overweight/#obesity #older patients with multiple comorbidities may seem intuitive, but it was confirmed by the scientific hierarchical cluster analysis, providing a potential technical rationale for the further development of more advanced and nuanced risk stratification implementations. Additionally, our study demonstrates that protein thiol groups (#PSH) partially might contribute to the high-risk mortality in overweight/obesity older patients. Given the potential of PSH in assessing #oxidativestress status and predicting all-cause mortality, further studies are warranted.
Interestingly, #young group population overweight/obesity does not significantly increase the risk in the long-term follow-up, thus providing a basis for stratified management of the risk of all-cause #mortality.

Another critical analysis based on #LIPIDOGRAM cohort studies with >45,000 #patients (median follow-up 15.3 years).
What we showed?
👉 Long-term all-cause #mortality (ACM) was lowest for the normal-weight group (14%) compared with other categories.
👉 The highest risk of #death was observed for the class III #obesity and #underweight categories (HR 1.79, 95% CI 1.55–2.05 & 1.57, 1.22–2.04), respectively.
👉A decrease in #bodyweight by 5 and 10% over 2 years was associated with a significantly increased risk of death during long-term follow-up—HR 1.45 (1.05–2.02, p = 0.03) and HR 1.67 (1.02–2.74, p < 0.001). Patients who experienced weight loss were older and more burdened with #comorbidities (unintentional weight loss).
👉 In nonsmoking patients without comorbidities, the lowest mortality was observed in those with a BMI < 25 kg/m2, and no #Ucurve #relationship was observed.

Great editorial by @m_piepoli and colleagues on the recent issue of #EJPC dedicated to exploring the relationship between type 2 #diabetes, #metabolicdisorders, and #cardiovascular #risk, including our next analysis from the #LIPIDOGRAM cohort #studies 2004-2015.
In this analysis we revealed that #MetS is associated with increased all-cause #mortality risk in #nonobese and #obese #patients. In patients without MetS, #obesity remains significantly associated with mortality. The concept of metabolically healthy obesity should be therefore revised. See:

Our critical analysis, which may in fact overturn the our approach to the #metabolicsyndrome (#MetS). Based on the data of >45,000 individuals from the #LIPIDOGRAM cohort studies we showed that MetS is associated with increased all-cause #mortality risk BOTH in non-obese and #obese #patients.
However, what is critically important to emphasize, in patients without MetS obesity remains significantly associated with mortality.
