#NeoCheckRay just published in @NatureMedicine - technically negative. Easy to scroll past. Don't. ER+/HER2− #breastcancer is the most common subtype. Immune-cold. In prior phase III trials, adding immunotherapy to chemotherapy helped, but only in PD-L1-positive tumors. The PD-L1-negative majority, roughly 60% of patients, gained almost nothing. Neo-CheckRay asked: what if radiation could change that? Precise stereotactic radiation to the primary tumor ( sparing the lymph nodes) to inflame it, then add a checkpoint inhibitor while the immune system is primed. The results: PD-L1-negative: pCR 3.4% → 29%. 25 points in patients assumed to be non-responders. PD-L1-positive: no difference between arms. Already inflamed?? probably radiation added nothing. Paired biopsies confirmed it within one week of radiation: cold tumors converting, immune pathways turning on. Radiation wasn't just treating the tumor. It was creating the conditions immunotherapy needs. Phase III needed. Survival data pending. But PD-L1-negative ER+ breast cancer may not be immunotherapy-refractory.
