How can #PBMs get away w/ DIR fees & other forms of PBM "extortion?" Because as this chart from @DrugChannels shows they are increasingly a monopoly (Or mob-opoly). Top 3 control 77% of Rx claims & top 6 virtually 100%. #PBMsMustGo
