2025 Australasian Bronchiolitis Guideline – What’s new?

PREDICT has just released the updated bronchiolitis guideline for infants <12 months. Here’s what’s changed since 2016 and what it means for your practice

(2/8) Risk factors expanded
In addition to the 2016 list, new risk factors include:
• Trisomy 21
• Congenital diaphragmatic hernia
• Prenatal tobacco exposure
• Economic disadvantage
• Genetic disorders
These are to be considered on a continuum, not binary.
(3/8) Severity scores
Still not recommended.
Formal bronchiolitis severity scores shouldn’t guide admission or predict LOS. Clinical judgement + risk factors matter more.
(4/8) Safe discharge refined
Now includes clear SpO₂ targets:
• ≥90% for most ≥6 weeks
• ≥92% if <6 weeks or has comorbidities
Also: feeding ≥½ usual volume, parental confidence, and social support are key.
(5/8) Steroids & adrenaline
Big shift:
• 2016: Steroids/adrenaline not recommended at all
• 2025: Still not for routine care, BUT
→ May be considered in ICU for infants with severe bronchiolitis needing advanced support (conditional rec).
(6/8) Oxygen & HFNC
• Use supplemental O₂ if SpO₂ persistently <90% (or <92% for youngest/high-risk)
• High-flow: not first-line for moderate disease, but can be considered if low-flow fails
• Avoid routine continuous oximetry if stable
(7/8) RSV prevention added
• Consider monoclonal antibodies (nirsevimab/palivizumab) for high-risk infants
• Consider universal antenatal RSV vaccination
• Routine infant RSV immunisation not currently recommended
(8/8) Summary
The 2025 update sharpens discharge criteria, broadens risk assessment, and cautiously expands ICU treatment options.
More nuance. More evidence. Still no to routine meds.
