Let's understand PLAN A, B, C, D blocks concept
Plan A Blocks: A Pragmatic Foundation
Definition:
Plan A blocks refer to a small group of basic, versatile ultrasound-guided nerve blocks that are:
Simple to learn and perform
Sufficient for providing analgesia for the majority of common surgical procedures
Meant to increase access to RA, especially for non-expert anaesthetists
Goal:
To create a practical set of go-to blocks that any general anaesthetist can reliably perform, improving equity and efficiency in regional anaesthesia delivery.
Plan B, C, D Blocks:
Advanced and Specialized Options
Definition:
Plan B onwards represents escalation in complexity or specificity. These are often performed by more experienced regional anaesthetists and are used when:
Plan A blocks are contraindicated or ineffective
Higher analgesic coverage is needed
More complex surgical requirements are present
Key Features of Plan B:
Require more anatomical knowledge
Potentially higher risks or technical demands
Not always necessary for every patient
Important tools in the hands of experts for *tailored* analgesia
Bottom Line
Plan A =
A few blocks for the many.
Aimed at widespread access, competence, and value in daily practice.
Plan B/C/D=
Many blocks for the few.
Reserved for complex needs, performed by RA specialists.
Examples:
Upper Limb : SHOULDER
Plan A:
Interscalene brachial plexus block
Plan B/C/D:
Superior trunk block, combined axillary and suprascapular nerve blocks
BELOW Shoulder
Plan A:
Axillary brachial plexus block
Plan B/C/D:
Infraclavicular block, supraclavicular block
Lower Limb: HIP
Plan A:
Femoral nerve block
Plan B/C/D:
Fascia iliaca block,
lumbar plexus block
KNEE
Plan A:
Adductor canal block
Plan B/C/D:
Femoral nerve block, IPACK block
FOOT and ANKLE
Plan A:
Popliteal sciatic block
Plan B/C/D:
Ankle blocks, proximal sciatic nerve block
TRUNK : Chest Wall
Plan A:
Erector spinae plane (ESP) block
