π· AMU rotation days 2-4
π· Drug histories and reviews for lots of patients with COVID, exacerbations of asthma, falls and many other conditions
π· Had a renal in house tutorial
π· Had a successful 13 week review!

π· Supposed to be day 1 of my AMU/A&E rotation, but I was needed and more than happy to help with drug histories on a variety of wards! It is all about being flexible and adaptable to change after allπ

π· Handled OTC queries and counselling patients with a variety of prescriptions - mainly antibiotics, steroids and creams
π· Attend my first virtual MMAG meeting and spent all week writing the minutesπ

π· Learnt that HIT (associated with LMWH) is most likely at days 7, 10 and 14
π· Made an intervention on a Paracetamol overdose in a patient under 50kg
π· Recommended pausing Atorvastatin due to raised ALT

π· Made an intervention on antibiotics prescribed for epididmyo orchitis (wrong dose and duration)
π· Learnt that bisphosphonates are renally excreted (avoid below eGFR of 35)

π· Learnt that IV Furosemide is twice as potent as oral
π· Learnt about transplant meds e.g. the usual Tac, Mycophenolate and Pred plus extras if needed like PPIs, Co-Trimoxazole and Nystatin, etc!

Be curious about a patient's concerns. Even if you have to tell them that these new headaches ARE related to their new drug. When they're back weeks later, saying "I wanted to ask you something first before I see my GP..."

Week 9 summary: This week i did my technician duties training. Did my assessment and passed so can now counsel patients on my own without supervision.
Came across a drug called Patiromer which I have never heard of before. It is used in treatment of hyperkalemia

π· Learnt that Calcium Carbonate, Insulin and Glucose are given to treat severe hyperkalaemia
π· Learnt that Loperamide has a risk of QT prolongation
π· Learnt that red man syndrome can occur with Vanc

π· Learnt that Alfacalcidol requires Ca monitoring to check it isnβt raised (and this is the opposite for Cinacalcet)
π· Learnt that Nitrofurantoin can cause peripheral neuropathy so is avoided below eGFR 30
