Wonderful summary by @WallisCJD of the importance of social media in urology for sharing evidence-based information, consuming new research & networking! Thanks for the shout out to @PCF_Science and #prostatejc! #urologyupdate21 @CanUrolAssoc

@PCF_Science BRCA2 patients clearly at higher risk of aggressive prostate cancer. BRCA1 likely also at increased risk of prostate cancer. Best evidence comes from IMPACT trial. #prostatejc

@PCF_Science An important research direction is to examine patient reported outcomes with germline testing. Please refer patients who have undergone genetic testing for #prostatecancer to join the PROGRESS registry #prostatejc @vedangiri https://t.co/brfohlRSKQ
@PCF_Science ⬆️ risk of clinically significant #prostatecancer and younger age at diagnosis in BRCA2 carriers ➡️ consider lower PSA threshold for further diagnostic workup (eg, markers, MRI, biopsy). Nice paper by @heatherhcheng et al #prostatejc

@PCF_Science 1⃣ An increasing number of men, often through cascade testing, are being diagnosed with pathogenic germline variants with an increased risk for prostate cancer. Coordination from providers across specialties will be key to helping them understand their risk.

@PCF_Science Thanks for selecting this article! Recommendations from NCCN and Philly Consensus meeting suggest starting at age 40, potentially tighter (e.g. annual) screening intervals depending on PSA levels.
@Ecastromarcos @SessineMichael We are generally not, largely due to lack of insurance coverage with non-elevated PSA. However, working with @NCIBillDahut @Dr_RaviMadan @heatherhcheng & team, with protocol currently open at @theNCI that covers MRI (as well as travel costs): https://t.co/UuMh12ReaA #prostatejc
Q2: What is the current state of the evidence on #ProstateCancer screening for patients with #BRCA2 and other pathogenic variants?
#ProstateJC
Patients had pathogenic germline variants in BRCA1, BRCA2, DNA mismatch repair genes (Lynch Syndrome), and CHECK2, among others. A total of 12/45 patients (26.7%) underwent biopsy. Of the 12, there were three with GG1 prostate cancer, one with GG2 prostate cancer.
There are many unanswered questions when it comes to prostate cancer screening strategies in these men (and avoiding over-screening). While our clinic highlights one strategy, the NCI (NCT03805919) highlights another.
What other strategies have you seen/used?

@PCF_Science As part of research protocol, we have started at younger age (35) and used lower PSA thresholds, and urine biomarker. #prostatejc

Poll for medical oncologists: what is your practice regarding germline genetic testing for #prostatecancer? #prostatejc
Poll for urologists: what is your practice regarding germline genetic testing for #prostatecancer? #prostatejc
What are the main barriers to genetic evaluation in your clinical practice? #ProstateJC
@PCF_Science Barriers to germline testing for #prostatecancer at multiple levels: variable interest among patients, differing views on genetic guidelines, variable availability of genetic counseling & ease coordinating care, concerns about legal & financial implications of testing #prostatejc
@PCF_Science Germline genetic evaluation is important for #prostatecancer management (e.g., PARP inhibitors) & clinical trial eligibility, as well as personalized cancer risk accessment & screening recs for patients and their family members- concerning that it is underutilized #prostatejc
Our next 48-hour #ProstateJC starts tomorrow, 8/3 at 10am ET! Please join!
We will be discussing the paper "Barriers and Facilitators of Germline Genetic Evaluation for #ProstateCancer" by Drs. @LoebStacy @vedangiri @randallLiMD @heatherhcheng @profmassey
