In advanced #ovariancancer, achieving complete macroscopic #cytoreduction (R0) remains the most important prognostic factor, as strongly emphasized by both #ESGO guidelines and #PSOGI #PelvEx consensus statements.
This patient had pelvis-confined peritoneal metastases, a pattern that often requires meticulous anatomical dissection and multivisceral procedures. Cytoreduction included en bloc pelvic peritonectomy, TAH/BSO, rectal resection, followeb by extended pelvic–paraaortic lymphadenectomy.
These steps are consistent with recommendations, which highlight the need for comprehensive compartment-based surgery when peritoneal surfaces of the pelvis are involved.
Because the disease was limited to the peritoneal cavity and complete cytoreduction was achieved, #HIPEC was also performed.
Key principles from PSOGI-ESGE-PelvEx consensus literature support its use in this context:
🔹 HIPEC may offer added benefit after R0/near-R0 cytoreduction in selected epithelial ovarian cancer patients.
🔹 A distribution- and biology-driven understanding of peritoneal metastases, where the extent of surgery is determined by the exact anatomic compartments affected.
🔹 Pelvic peritoneal metastases frequently require en bloc removal of affected peritoneal surfaces, fully in line with peritonectomy principles.
🔹 In accordance with PelvEx principles, when tumor biology and anatomical distribution require it, pelvic exenteration should also be considered to achieve complete cytoreduction.
These operations rely on a true multidisciplinary surgical model—including general surgery, gynecologic oncology, and, when required, urology—working in coordination with the medical oncology team. Accurate staging, operative planning, and postoperative management are further supported by radiology, nuclear medicine, and pathology, each contributing essential diagnostic and decision-making insights.
This case illustrates the extent of surgery in ovarian cancer is shaped by the disease’s anatomic spread, not by surgical preference.
When appropriate, combining cytoreductive surgery with HIPEC aims to optimize long-term oncologic outcomes.
