Thx #CardioX for all the input (#RemoveIt, #PushIt, #CallSurgeon, #RunAway,...). Here is my #Solution:
I choose the option to do a proper desinfection and investigation in the cathlab. After desinfection the collagen sponge seems to suck a the skin level and no incission was obviously done at the index procedure. I did a smal incission medial and lateral and did a little preparation of the subcutaneous tissue to free the sponge and get a better look with the aim to cut the collagen sponge as much as possible.
BUT: When I tried to lifted the collagen sponge with a clamp the Angioseal (collagen, string and the anchor) came easily out (#AllinOne). There was only little bleeding mainly caused due to the expansion of the incision and not coming from the femoral artery. After short compression bleeding stopped.
The anchor was strangly V-shaped and IMHO was never intraarterial and always external to artery. It is strange to me that it seal the puncture site with no external bleeding.
I informed the collegue about the trouble with two major learnings:
- Always cut the skin before placing an AngioSeal
- Never discharge a patient with such a mess and train the staff to check puncture site
- Go #radialfirst
