In adults ≥70 without known CVD, atorvastatin 40 mg reduced major CV events (6.0% vs 8.3%; HR 0.70) but did not improve disability-free survival. Clear CV benefit, with treatment individualized to risk and preference.

Absolute benefit, frailty, polypharmacy, preferences and adherence still matter. CV prevention improved; healthy longevity did not. #STAREE
