🔸 As we near the end of #16Days, we reflect on a harsh truth: mothers and children who have separated themselves from their abuser are #StillNotSafe. Through child contact proceedings, abusers can continue to control, threaten, and harm families 💔, while Scotland’s courts and Children’s Hearing System too often enable this abuse ⚖️.
The gap between criminal and civil courts, poor risk reflection in social work and police reports, and a bias towards contact leave survivors vulnerable, even as research shows the harm this causes. Scotland can, and must, do better. It's time to reform our systems, protect children, and truly prioritise the safety of survivors.

Libraries of evidence, decades of case studies, and we still have a civil and criminal justice system that enables abusers to control, threaten, and abuse children and their mothers through contact and visitation hearings. Separation ≠ safety. #StillNotSafe
Today is the first day of #16Days and the International Day for the Elimination of Violence against Women. To mark this day we are launching our #StillNotSafe child contact campaign.
During #16 days and beyond we will be pushing for reform of the systems that deliver damaging child contact decisions and make women and children unsafe and re-traumatised.
We will advocate for change, implementation and reform across Government, justice and legal systems, social work, and support services. These decisions result in abuse against women and children after they are supposedly free of their abuser. They are not. Over #16Days we’ll share the experiences of those who have been through the system. It’s time for reform. #StillNotSafe #16Days

Indeed. Abusers repeatedly target mothers in civil courts and Children’s Hearings in Scotland, in custody and visitation actions. Abusers’ motives? Avoiding child maintenance; exploiting her biggest fear “I will take the children away”; control, control, control. #StillNotSafe
It is not only journalists and newsrooms who are at risk of being silenced when it comes to the use of abusive lawsuits
Apsana Begum shared her experience of lawfare as a victim of domestic abuse today in the House of Commons

It can't be errors alone. The misperceptions about patient safety by people who know nothing at all about it and aren't interested in learning about it is taking decades to end. #stillnotsafe
@nosecglasses @NormanMacleod14 @ThomasJohnRose @StevenShorrock @DeakinSue1 @nialldowney @KenZeroHarm @PatrickWaterso1 @CIEHF @MartinBromiley Like many clinicians, it suits them not to know. If 'errors' are only a human / professionalism problem, then we can safely dissociate them from financial decisions. With this training, we no longer even have plausible deniability. #StillNotSafe
@MattGoldis @PatrickWaterso1 The focus on behavior and CRM has been used to hide deeper problems (e.g. "Situational Awareness Failure" rather than looking at where SA comes from), so in healthcare it's entrenched the ways in which accidents happen, leading to more of them. No doubt. #stillnotsafe
@nialldowney @DeakinSue1 @doctorcaldwell @NeelamDhingra1 @WHO @DonaldsonLiam @TheIHI ...and after 20+ years, that's not an accident. #stillnotsafe I think about the number of people who might still be alive who aren't because the HF=CRM fixation has helped perpetuate "blame and retrain". If I forget my manners sometimes, it's because of that.
@ChrisElston1912 @DrPaulSalmon Sadly, some of our clinical colleagues claim to be HF experts while deliberately ignoring anything but NTS, and are dismissive of anyone who dares to challenge their ignorance. Repeat that for over a decade and we have lots of unnecessary harm. #stillnotsafe
@DrGetafix @hypoxicchicken @dasairway Would have been faster if we’d employed qualified HF experts to work alongside clinicians 10 years ago. This still isn’t taken seriously enough. #stillnotsafe.
@Mike_Durks @SusannaStanford @hsib_org @Imperial_IGHI @PLAN4ZERO @Jeremy_Hunt @WHO_GPSN @NHSEngland If you want to improve safety you need to employ safety scientists, including systems engineers, and human factors specialists. Clinicians can’t do it alone. That’s been the biggest mistake over the last 20 years. We see the same misconceptions repeated. #stillnotsafe
@KenCatchpole I’m listening to #StillNotSafe (audiobook). It validates so many of my trepidations during nursing school about the IOM report and risk of vilifying practitioners by talking about medical error in this way. So very glad to read Wears’ work having missed out on meeting him irl.

Re: accountability - will only be starting to be appropriate when we see CEO/CFOs implicated. #stillnotsafe
