2/ All three techniques require visualization of the distal TL #3DW → retro injection, marker wire #TDADR → IVUS image of dTL #ADR incl. Stingray → retro injection ALL three require wires with a) torque control and b) penetration power. Here is an image from last year.

On angiography, the guidewire is pointing downward—and the IVUS image also shows it heading downward. Synchronizing these two views like this is crucial for safe and accurate wire manipulation. That’s the essence of the “be the wire” technique.

After the @crfheart CTO meeting each year, I usually make practice adjustments based on the dense 2 days of content! Congratulations to this year's directors, and looking forward to next year's warmer one in Miami! @mbmcentegart @JWMoses These are my #CTO2026 take-home messages! Starting with small, quick tricks 1- A discussion on engaging septals from the PDA- Use tip injection in the PDA, engage the collateral with "Sion" (for me) rather than Suoh, then can exchange inside the collateral into Suoh given extreme tortuosity coming from the RCA side (Shunsuke Matsuno, @docjohnnyg ) 2- A nice trick by @RinfretStephane to avoid pistoning of retrograde microcatheters is to anchor with a small balloon in the donor vessel while wiring, then deflate the balloon. Gotta be careful of donor vessel injury or ischemia. 3- Utilizing a cutting balloon for tough side base/base when not able to start a knuckle. I knew about it and never tried it. Will try it in 2026 @Billlieofficial 4- TD-ADR, the concept that is tough to implement, especially with the lack of certain tools in the US! This time, I learned we can do it with 5-Fr Opticross! So I am hoping to try it in 2026 The concept depends on moving the IVUS (back and forth) to visualize a mental 3D image of the relationship between the guidewire tip and the true lumen and attempting to puncture it! That slide helps better evaluate it (@Laserrman Maoto Habara) 5-I use CTA in about 60% of my CTO PCI procedures planning (ambiguous caps, ambiguous course, prior CABG, prior failures). After this conference, knowing what's coming in terms of software ability, I think my utilization of CTA will significantly increase @ColletCarlos (1/3) #CardioX #ACCFIT

These are my #CTO2026 take-home messages!
Starting with small, quick tricks

1-
A discussion on engaging septals from the PDA-
Use tip injection in the PDA, engage the collateral with "Sion" (for me) rather than Suoh, then can exchange inside the collateral into Suoh given extreme tortuosity coming from the RCA side (Shunsuke Matsuno, @docjohnnyg )

I did not get the impression that the article is arguing against niche or investigational procedures. Rather, it challenges how we use them in everyday practice. #TDADR is a technique, so it is not the target - but we can (and should) scrutinize how #CTOPCI is performed in the community. PROCTOR reminded us that conventional wisdom can be wrong, and INVEST-CTO reinforced that sometimes less is more - both consistent with the theme of the editorial.

@JoySanyal74 Our #TDADR attempt got us into the RV branch but on the opposite side of the TL behind Ca. 😢 We failed RTIG CTO re-entry → so did blocking balloon assisted STAR for the PLB and CBA AFR for the PDA. But we restored antegrade flow! Note the ‘missing’ left 👉🏼 right collaterals!

New Year, New Resolution! Here is my resolution for 2026! Implant less metal! 🤞🏼 Here is a case from this week - failed RCA #CTOPCI at OSH. We were also unable to cross despite multiple #HDR, #TDADR, retro attempts. Settled for IVI guided, CBA facilitated STAR! Will f/u with PET in a few weeks.

#CTO #HDR #TDHDR #TDADR Pilot200, GAIA 2nd, Conq 12→#HDR→EP→#KWT→#TDHDR→Type 2A stains→sionblack failed to cross→#KWT→#TDADR→Try to puncture several times→Conqest made vessel perforation and hematoma→Finally get DTL at #3 proximal portion. A penetration guidewire can easily perforate the vessel and go outside, causing an extravascular #hematoma.
