β-#thalassemia_minor is the most common #autosomal_recessive disorder of #hemoglobin, worldwide. Diagnostic tests are #CBC, #HPLC, #Hemoglobin_electrophoresis, #Ferritin, #Transferrin_Saturation, #TIBC, #PF. There are usually #mutational_defects in #beta_globin_gene which leads to defective or decreased beta chain synthesis. Clinically it is a very heterogenous disorder and ranges between asymptomatic carriers to moderate or severely #anemic patients. #Blood_work reveals low MCV (#microcytosis) MCH, and MCHC (#hypochromia) with or without #erythrocytosis. Further work up shows a raised Hemoglobin #HbA2, a hall mark of its diagnosis. Isolated or combines #nutritional_deficiencies considerably alter the red cell indices and morphological patterns such as #pancytopenia, #macrocytosis, and #erythrocytopenia. Thalassemias and #iron_deficiency anemia (IDA) are the two most common etiologies of microcytic hypochromic anemia in children and adults. It has long been considered that iron deficiency does not exist in thalassemias, including thalassemia major as well as minor. However, studies have shown the occurrence of iron deficiency in patients with β-thalassemia minor. Lower initial hemoglobin levels in patients with coexisting iron deficiency anemia and β-thalassemia minor are shown as compared to the patients of uncomplicated β-thalassemia minor. This has been explained by the lack of hematopoietic nutrients due to iron deficiency superimposing on the imbalance in globin chain synthesis. Similar changes have also been shown in other red cell parameters, serum iron, #ferritin, and total iron binding capacity #TIBC. These changes have also been demonstrated to improve after adequate iron replacement therapy. HbA2 levels have been reported to be lower in patients with coexisting iron deficiency anemia and β-thalassemia minor, with improvement in levels after iron therapy. However, other studies have shown no significant difference in HbA2 levels in such patients. The reduction in HbA2 levels in patients with concomitant β-thalassemia minor and iron deficiency anemia leads to the misinterpretation in the diagnosis of the former. A recent study has hypothesized that such an occurrence can lead to these patients with β-thalassemia minor marrying another person with β-thalassemia minor with risk of birth of thalassemia major child. Usually, cases of β-thalassemia minor show raised RBC count, microcytosis, and a normal RDW. Classically, β-thalassemia minor is diagnosed as a raised (>3.4%) hemoglobin A2 level along with or without microcytosis and raised RBC count. Hemoglobin A2 (HbA2) is a normal variant of adult hemoglobin that consists of two alpha and two delta chains and is found at low levels in adults. Clinically, β-thalassemia minor presents as #pallor, #tachycardia, and #shortness_breath on exertion. Clinical features depend on the degree of anemia. Underlying secondary causes (if any) also contribute to the signs and symptoms.
