Skin biopsy: acute inflammation in dermis with fat necrosis.
MRI pancreas: acute-on-chronic pancreatitis with peripancreatic fluid collection.
Patient had knee arthritis with the rash.
Final diagnosis: PPP syndrome

@irenep671 I don’t know for sure as it was a #TwitterConsult. There are gastric erosions at the incisura with nodular gastritis (possibly GA/GIM) which suggests H pylori as the cause.
