2. To diagnose autonomic failure (AF), perform a Valsalva manoeuvre and watch for an acute drop in BP and the absence of a BP overshoot once the manoeuvre has stopped.
Drinking water is an effective treatment (causes a sympathetic rise in BP in these patients).
