My patient has 50seconds/8.3 breaths or 6 seconds per breath to get out 750cc. His expiratory airflow rate needs to be only 125cc/second meaning that if it's down to the wire, my patient will have more time to exhale the Ve and should have less #autopeep
To avoid #autopeep and its untoward effects, we need the lungs to empty faster, or the give them less air to empty. For the former, the constant "T" must change, but that requires abatement of the asthma flare w time. All we can do is try to give less air so there's less to empty
...a condition called #autopeep that puts the patient at risk of barotrauma and hemodynamic compromise, the former b/c of #hyperinflation, the latter b/c the gradient for venous return (Pms-Pra) is reduced as volume and pressure build in the chest. Blood can't return.

To set #ventilator for bronch, I increase Vt; this reduces #deadspace and allows reduced RR which reduces #autopeep risk. I raise #peakpressure alarm to 100 so the breath is not terminated prematurely. Most importantly I reduce #peakflow to minimize peak pressure
