Sometimes axillary vein anterior wall is too complacent. If you know exactly were your needle tip is, apply little pressure or twist the syringe a little as @50wattdoc first proposed. It helped me in some cases

Axillary puncture using caudal 35°.
Left pannel caudal 35°, right pannel AP. Look how in AP needle invades thorax, whereas in caudal 35°, always needle away from it. No dye needed never. Give a try.
I think #cephalicfirst and #axillaryfirst #EPeeps can argue until the cows come home about who’s approach is more advantageous. But I think everyone agrees that NOBODY should be sticking the subclavian vein first (on purpose at least). #LongUSguidedAxFirst 😄
