In a randomized trial in #CORR, Ham et al. found that a single epidural ropivacaine injection with opioid-free IV-PCA provided effective pain control, reduced opioid consumption, and decreased urinary retention risk after single-level lumbar spine surgery.
"These findings suggest that surgeons can effectively manage postoperative pain in these (opioid naive) patients without relying on routine high-dose opioid regimens, thereby minimizing the risk of opioid-associated complications," the authors write.
"Based on these findings, I believe that surgeons could easily consider a couple options. The first would be a bolus of local anesthetic proximal to the surgical field at the conclusion of surgery," writes Charles A. Reitman in a #CORRInsights commentary. "Secondly, the benefit of a nonopioid patient-controlled analgesia was rather striking. Adopting nonopioid analgesic options should help with faster transitions to oral regimens, fewer side effects, and ultimately faster discharges."

In #CORR, Tokuyasu et al. found that lumbar fusion after THA was associated with improved sagittal spinal alignment and low back pain, reduced lumbar motion, and increased compensatory use of hip flexion in the flexed-seated position, particularly in patients with a higher number of fused segments.
"Our findings suggest that obtaining flexed-seated radiographs preoperatively in patients with prior THA who are scheduled to undergo lumbar fusion may assist spine surgeons in communicating the potential risk of postoperative dislocation with patients and hip surgeons," the authors write.
"This (study) may encourage surgeons to revise their thinking on target version angles during index THA," writes Andrew Kurmis MD, PhD in a #CORRInsights commentary. "The move toward genuinely patient-centered surgical decision-making (rather than clinging to the ideal that one target cup position is best for all) should go a long way to improving patient outcomes."

In #CORR, Xiong et al. found that insertional reattachment for acute Achilles sleeve avulsion fractures provides meaningful and durable improvements in pain, function, and activity levels at least 5 years after surgery.
"Clinicians should be aware, however, that patients presenting with preinjury insertional pain or prior corticosteroid injection—features often reflecting underlying tendinopathy—may have a slower or less complete recovery," the authors write.
"This raises the question about prevention," writes Casey Jo Humbyrd MD, MBE in a #CORRInsights commentary. "There may be imaging findings that predict rupture or particular findings associated with tendon quality that could be incorporated into a scoring system that has prognostic capability."

In #CORR, Aepli et al. demonstrate how their novel frame-based draping technique for direct anterior approach THAs on a standard table reduced setup time by 10 minutes, surgical time by 9 minutes, and was associated with lower rates of both infection-related and overall revisions compared with other DAA THAs performed in Switzerland.
"Although some differences likely reflect institutional and surgeon volume rather than the drape itself, these results strongly suggest that frame-based draping is not inferior to traction-table strategies, and that it may streamline workflow for surgeons who perform DAA on a standard table," writes Geert Meermans MD in a #CORRInsights commentary.
"We believe that this approach could benefit all surgeons who perform the DAA, especially those still in the learning phase, who are at a higher risk for complications such as dislocations and leg length discrepancies," the authors write.

In a systematic review in #CORR, Samson et al. found that multiple aspects of social health—such as limited health insurance, unstable employment, and housing insecurity—are associated with greater levels of musculoskeletal discomfort and incapability.
