Therapeutic strategies in adult patients with relapsing and/or refractory cutaneous #IgA #vasculitis (French multicenter study, n = 52)
◦ In adults with relapsing or refractory cutaneous IgA vasculitis, the highest short-term skin response rates were seen with #dapsone (82%), #glucocorticoids ± #colchicine/dapsone (73%), and conventional synthetic-DMARDs (i.e. azathioprine, methotrexate, mycophenolate mofetil) (71%).
◦ Colchicine monotherapy was clearly less effective (57% response).
◦ Although dapsone had the highest response rate, it also carried the highest subsequent relapse rate (46%). Glucocorticoids-based regimens and colchicine had lower relapse rates.
◦ Seven patients received rituximab, including three as second- or third-line therapy. Among them, two achieved a partial response, while no response was observed in the remaining four patients.
◦ These findings suggest that dapsone appears to be an interesting and reasonable option for relapsing or refractory cutaneous IgA vasculitis in adults.

#Dapsone #Tablets #Therapeutic indications:
1) As part of a multi-drug #regimen in the treatment of all forms of #leprosy.
2) Treatment of #dermatitis #herpetiformis and other #dermatoses.
3) #Prophylaxis of #malaria in combination with #pyrimethamine.
in patients with methemoglobinemia, pts are not necessarily anemic & oxygen saturation is discordantly low compared to the pO2.
Methylene blue is an appropriate acute intervention for methemoglobinemia caused by #dapsone.
ddx #Dapsone can cause methemoglobinemia of sufficient degree to cause hypoxemia & fatigue/weakness but methemoglobinemia does not lead to anemia. In this case: oxygen saturation was unremarkable.
G6PD deficiency in pt on dapsone can cause severe hemolysis. Should test for G6PD deficiency prior to using #dapsone
In people of color: Hemolysis is more common but mild
In Caucasians: hemolysis is less common, but more severe
#Dapsone-related #hemolysis in a patient with glucose-6-phosphate dehydrogenase #G6PD deficiency given the anemia and high LDH.
