Efficacy and safety of intraoperative hyperthermic intraperitoneal chemotherapy for locally advanced colorectal cancer (HIPECT4): final analysis of randomized clinical trial

The long-term outcome analysis of the HIPECT4 trial including 184 patients demonstrated that using mitomycin C-based HIPEC reduced peritoneal recurrence in patients with locally advanced colon cancer without increasing toxicity. However, there was no difference in overall survival and disease-free survival.

@sukumar_vivek I don't believe so. Lymphatic disruptions cannot be altered with wider peritonectomies. Nor can you prevent retroperitoneal implantations

I have to say #HIPEC does not work nowadays for #CRC because biological reasons and evolution of systemic therapy. @CBCcirurgioes @sbco_oficial

On one hand I think #HIPECT4 does HIPEC +sCT in the experimental arm in order to protect from both #peritoneal and systemic risk. On thé other hand, we see accumulating evidence about the role of biology and CMS4 subtype. @SPeritoneum @alarjosan @me4_so @ISSPP1 @RENAPE_Network
Pushing the frontier in a difficult-to-treat disease: peritoneal carcinomatosis. @AndreaCercek highlights the importance of systemic therapy. Kudos to the HIPECT4 team for completing a randomized trial. We need to understand the biology of this disease better to beat it #ESMO2022
