Can cryotherapy prevent paclitaxel-induced peripheral neuropathy?
This investigator-initiated clinical trial was conducted at the National Cancer Center Hospital East (#NCCHE) and Cancer Institute Hospital (#JFCR) to evaluate a limb cooling device for the prevention of chemotherapy-induced peripheral neuropathy (CIPN).
🔸 Weekly paclitaxel in the perioperative setting for breast cancer
🔸 Limb cooling device used during each infusion
🔸 Double-blind, randomized comparison between 13°C and 25°C settings
🔸 Primary endpoint: incidence of moderate or worse neuropathy (PNQ ≥ D)
Results: No significant difference in the incidence of neuropathy between the two temperature groups.
This outcome was disappointing, especially as many participating patients reported favorable impressions of the device and felt it helped prevent symptoms.
The 13°C setting was expected to be effective, while 25°C was chosen as a control with minimal expected benefit—but still provided a sensation of cooling for blinding purposes.
Ironically, the 25°C group may have experienced some benefit after all.
Notably, patients enrolled during the summer showed a trend toward less neuropathy in the 25°C group.
It's possible that 25°C was sufficient under high ambient temperatures—or that excessive cooling at 13°C had negative effects.
Among patients in the 13°C group, those whose hand surface temperatures remained relatively high (i.e., insufficient cooling) tended to experience fewer symptoms, suggesting that "overcooling" may reduce efficacy.
Rather than concluding that limb cooling is ineffective, we believe the trial highlights the need to identify optimal temperature settings for different conditions—further research is warranted.
