Even in death, last rites and family access were refused. Denial of rights — Admission and consent forms were left unsigned or marked by staff as “unable to sign.” Staff falsely claimed “verbal” consent to treatments was provided. No informed consent — Victims/POA received no information on risks or side effects of medications, intubation, or procedures as evidenced by no formal documentation in medical records and testimony by family or survivors. The nursing homes during COVID is the part I cannot move past. The specific and systematic horror of what happened to the elderly in care facilities across the Western world in 2020 and beyond is the concentrated version of everything that was wrong with the institutional response to COVID. And it has received a fraction of the examination it deserves, partly because the examination would implicate the same governments whose policies produced it. These were people already living at the outermost edge of the social margin. Already removed from the texture of ordinary life by the combination of physical decline and the institutional infrastructure that manages that decline on behalf of families who are navigating the genuine difficulty of caring for people who need more than family structures can usually provide. They were already living with reduced contact. Already spending days in environments designed for management rather than meaning. And then the doors were closed. Not metaphorically. The families that came, sometimes daily, sometimes multiple times a week, who were the primary human connection these people had to the world outside, were told they could not come. The visits stopped. The contact that remained was mediated by glass and screens and the protective equipment of staff who were themselves frightened and overwhelmed and unable to provide what families provide. People spent months alone in those rooms. Disoriented. Frightened. Dying in some cases without understanding what had happened or where the people who came every week had gone. The cognitive decline that isolation accelerates in elderly people is well documented. The psychological cost of sudden, unexplained withdrawal of contact from the people who represent your primary human bond is not subtle or ambiguous. It is severe. It is, in some cases, lethal in its own right, separate from the virus the isolation was supposed to prevent. The governments that imposed the care home restrictions and simultaneously sent COVID-positive patients into those facilities bear accountability for what happened that has not been seriously pursued in any jurisdiction. But the broader question that is not being asked is what the acceptance of those policies tells us about the status of the elderly in Western societies. Whether the ease with which the restrictions were accepted reflected a pre-existing cultural arrangement in which the elderly were already, in some functional sense, managed rather than cherished. Whether the isolation of 2020 was a concentrated version of an isolation that was already operating more slowly and more invisibly before the emergency gave it institutional form. The nursing home is where the answer to that question lives. And we are not going there.
