60 y/o male with a 10 cm bladder mass.
High-grade urothelial carcinoma with extensive squamous differentiation.
-Presence of typical urothelial carcinoma/carcinoma in situ precludes a diagnosis of primary squamous cell ca.
-Extent of squamous diff >20%.

Studies have suggested that the loss of PAX 8 and ER expression is a fundamental feature of dedifferentiation. Cases of DEC may also lose expression of MMR proteins.

Myoepithelial cells are usually positive for p63, SMA, and calponin. Luminal-like cells are positive for CK7, CAM5.2, and CD117.
ACCs usually have good prognosis and are ER/PR/HER2 negative.
