Old yet important lesson: Not every thickened gall bladder wall on #POCUS = cholecystitis. Can be seen in any volume overload state. Here is an e.g. from an acute #heartfailure pt. Note the portal vein #VExUS showing to-&-fro pattern due to systolic flow reversal (congestion)

Thickened gall bladder wall + abnormal LFTs ≠ cholecystitis. These images are from a patient with #HeartFailure exacerbation and no abdominal pain. Portal vein is pulsatile with systolic flow reversal indicating severe congestion. GB wall thickening is due to CHF #POCUS #IMPOCUS

PAN-#POCUS case: pt w SLE and nephritis. Meds: CellCept, PDN, HQN. Comes w cough and SOB. HR 120, BP 90/56, RR 34, O2 70% RA. Lactate 9, Cr 5.6 mg/dl (baseline 2.1), WBC 3.2, Plt 73. Blirrubin 8 (Direct 5), INR 1.5. Recieved 1 L crystalloid on ED and antibiotics. #IVC shown:
