Alberta just released draft legislation (Nov 18, 2025) that would allow physicians to work in both the public system and private clinics at the same time – creating a dual-practice, two-tier healthcare model. If this sounds familiar, it should.
The UK has been living this reality since Brexit-era staffing shortages forced the NHS to outsource a record 659,000 inpatient treatments to private providers in 2024 (up 50 % in just a few years) and spend £12.4 billion on independent sector contracts.
The British results?
· Waiting lists still at 7.4 million.
· Public-sector vacancies unchanged or worse.
· A growing “two-track” system where simpler, profitable cases go private and complex care is left behind.
· Regional inequities and training gaps widening.
Alberta’s government says this will reduce our 28-week specialist waits and give doctors “choice.”
Experience from the UK (and earlier experiments in BC and Ontario) strongly suggests the opposite: the most experienced physicians migrate to higher-paying private work, public hospitals lose talent, costs rise, and access becomes tied to ability to pay.
