#ComplexPCI Another option (highlighted by the great @LAzzaliniMD) when facing an #uncrossable #calcified lesion. Finecross stuck at the lesion and #RotawireDrive crossing distally. Then #Rota, #OCT, #IVL (based on the OCT findings) and overlapped DES. #Rotatripsy #DEBSCAN_IVL

When U perform #UltraLowContrast PCI in a moderate-severe lesion based on an angio performed in other center some time ago…and you have a 😮 #Uncrossable #Calcified #ISR treated with #ELCA+#DEB GFR12 ml/min. Total contrast vol 4cc. MSA 7mm2. #Triregistration #iFR #IVUS

1:1 NC 🎈expanded well (2 ortogonal views)➡️overlapped DES were implanted with good result confirmed with IVUS. Several ways to solve this case (poll results) but: ➡️ #Intracoronary imaging is useful in #ComplexPCI ➡️ #ELCA could be a safe and effective tool in calcified lesions

2nd attempt 2 weeks later. Femoral+7F long sheath+AL2+ GEC. Disection remains. #IVUS does not cross. Small 🎈crosses but severe #underexpansion. From #uncrossable lesion to ➡️ #undilatable lesion. ⬇️🧵

Case from @Hemodin_LaPaz by @AJuradoRoman 53 yo male, #NSTEMI, normal LVEF, severe distal Circ. Some tortuosity. No severe angiographic calcification. Ad hoc PCI, radial 6F and surprise😅…#uncrossable lesion even with GEC. Furthermore, Dissection during the procedure. 🧵⬇️
