Although #bisphosphonates are used to treat acute #osteonecrosis, reducing pain and helping to prevent disease progression and #bone collapse, long term use of bisphosphonates has also been associated with the development of osteonecrosis in sites other than the jaw. #Zoledronic acid is associated with a higher risk of osteonecrosis than less potent bisphosphonates, and this risk gradually decreases after the end of therapy, however, it is still increased >5 years later. Treatment with #denosumab is also associated with an increased risk of osteonecrosis.

IV #bisphosphonates (i.e. #zoledronic acid, #pamidronate) are used in the treatment of #hypercalcaemia of malignancy in adults, however, the current evidence and guidelines suggest using #denosumab in preference to IV bisphosphonates as first-line pharmacological treatment in these cases:
◦ Although comparative studies of IV bisphosphonates and denosumab did not report treatment of hypercalcaemia of malignancy as an endpoint, the results of indicate that denosumab is a more potent suppressor of bone turnover and, therefore, is expected to be more effective.
◦ Side-effect such as infusion reactions and renal injuries are also more common with IV bisphosphonates than with denosumab.
◦ Denosumab has additional advantages in that it can be administered subcutaneously in community settings and can be used in stage 4+5 CKD, albeit with a higher risk of #hypocalcaemia.
