@MOHOspark @Feeeeeeebr1 Person to be involved at the right time, to make sure our service is the best suited for the patient needs, if it’s presented through MOHOST with all physical needs and minimal cognitive concerns, ?would a rehab service or social work be more appropriate (2)

@MOHOspark @Feeeeeeebr1 I think it’s about us having all the right info at the start, ensuring we’ve tapped into every corner of OT, and that just streamlines input going forward and care planning. It also brings together patients varieties of needs and ensures we are the right (1)
