Feeling nervous when it comes to nerve imaging?
MR neurography is becoming standard
If you don’t know how to read it, it can be nerve-wracking!
Here’s the cheat sheet you NEED to be able to read these studies!

Here's an easy way to remember the different injury grades on MR neurography:
➡️Class I injury: Neuropraxia
🔸Myelin is injured, but nerve is intact
🔸Nerve is bright on imaging, but normal in size
🌟Remember:
▶️Roman numeral 1 (I) looks like the normal axon of a nerve. Nerve looks normal on imaging except for increased T2 signal
➡️Class II/III injury: Axonotmesis
🔸Axon disrupted, but connective tissue intact
🔸Nerve is both bright & enlarged
🌟Remember:
▶️Roman numerals II & III look just like Roman numeral I but thicker. Nerve is both bright & thickened
➡️Class IV injury: Neuroma in continuity
🔸Only endoneurium is intact
🔸Nerve has focal thickening/neuroma
🌟Remember:
▶️Roman numeral IV is the Roman numerals I & V. Roman number I looks like a nerve & Roman numeral V looks like a sideways scissors. So IV is a nerve that is almost cut completely by scissors
➡️Class V injury: Neurotmesis
🔸Nerve is severed
🔸Nerve shows discontinuity
🌟Remember:
▶️Only the V or scissors remain. No Roman numeral I here for a nerve, so nerve is completely severed & only the scissors remain
Hopefully, these tricks strike a nerve & help you remember the findings of nerve injury on imaging!
With this cheat sheet, you won’t have any trouble getting up the nerve to read MR neurography!
