32/M, ChBCS, Recurrent UGI bleed. Fibrosed Major Hepatic veins, referred for DIPSS. Dilated Right Inferior Hepatic vein with ostial Web identified on USG/CT.
Venoplasty of accessory HV done. No further GI bleed at 3 months follow-up with Improved QOL.

Cannulate LHV ➡️ Insert RUPS over wire ➡️ USG probe in midsagittal plane in epigastrium ➡️ rotate stiffening cannula to bring it in the same plane as USG probe ➡️ PV puncture ➡️ stenting
