🕯️ integration of palliative care and clinical ethics
⛔️ preservation of patient autonomy when institutional or benchmarking pressures influence care
🚧 responding to requests to continue ECMO when there is no exit strategy
Ethical tension of #ECLS may generate moral distress for clinicians, patients, and families: here recommendations to emphasize transparent, team-based decisions, early involvement of ethics and palliative care, and consistent processes for ongoing review of candidacy and continuation of ECMO.

#ECMO in patients with traumatic brain injury and/or requiring neurosurgery: outcomes, complications & management considerations
🧠 TBI should not be considered an absolute contraindication: #ECLS can be safely utilized. Successful outcomes relied on collaborative efforts + minimally intensive anticoagulation. A subset of patients developed ICH on ECMO (TBI/not), requiring neurosurgical interventions: key factors affecting prognosis included ICH size, timing of intervention, careful adjustment of anticoagulation. Discontinuing ECMO prior to intervention may be advantageous if clinically feasible.
