@luke_hedrick Only thing I might add is that Uosm is not a perfect proxy for ADH. Can be high because the kidneys are dumping other solutes out, e.g. urea when recovering from an AKI. Can estimate the electrolyte-free water clearance in these scenarios

I’m looking forward to seeing the full results of EMPA-KIDNEY which included pts with:
-GFR down to 20
-UACr below 300
ACE inhibitors and ARBs are not “nephrotoxins.”
They improve renal blood flow.
But they reduce GFR, and impair potassium elimination.
Which is why we might hold them in [severe and/or worsening] AKI.
