EMS call a STEMI alert based on their prehospital ECG. You review it and there's STEMI criteria. However, the ED ECG has no STEMI.
Do you:
(a) Tell the EMS they are bozos that don't know how to obtain an ECG, or
(b) Proceed with STEMI activation protocol
Anecdotally, this seems to come up all the time. The question is whether this is artifact or the patient has a major lesion that may have auto-lysed.
In this study of >24,000 prehospital ECGs with 293 STEMIs. There were 22% that had resolution of the STEMI on the ED ECG.
Among those that had persistent STEMI findings, 85% of them had a lesion of >/=95% on cardiac cath. Comparatively, among those with resolution of the STEMI, 63% of them also had a lesion of the same magnitude.
Therefore, while the resolution of the STEMI findings confers a 26% lower relative risk of a total or near-total lesion, this risk is still substantial. This suggests that among those who likely auto-lysed en route, there is still a high probability of finding a significant lesion.
