It’s a subtle reference to a real phenomenon in Japan: because of universal healthcare, prescription medications such as painkillers and psychiatric drugs can be obtained relatively cheaply, and some people resell their leftovers for extra cash. Pay attention to the details—the creators put a surprising amount of social commentary into the background.

For example, the scene where they’re inhaling drugs from a balloon comes from South African. The creators worked with journalists and researchers who have spent years documenting slum cultures across the world, drawing influences from many different countries rather than just the United States.
