A woman in her 40s w/ a 4-yr hx of Hurley stage III HS presented w/ chronic drainage from the axillae, inframammary folds, & pelvis, along with a 2-mo hx of this painful ulcerating violaceous nodules on both lower extremities. She had no hx or clinical evidence of severe acne/acne conglobata.

Diagnosis: Pyoderma gangrenosum (PG) occurring w/ hidradenitis suppurativa (HS).

PG & HS are chronic inflammatory dermatoses that may coexist because of shared autoinflammatory & neutrophilic pathways. This patient w/ Hurley stage III HS developed a 2-month hx of painful ulcerating violaceous nodules on both lower extremities, a presentation highly suspicious for PG. The absence of severe acne/acne conglobata made PASH syndrome less likely. Bx supported the diagnosis of PG.

She had transient improvement w/ systemic corticosteroids but flared after discontinuation. Because prior hepatitis B exposure made TNF-α inhibition less desirable, alternative biologic therapy was pursued. Secukinumab & upadacitinib were ineffective or poorly tolerated.

Tx w/ bimekizumab (dual IL-17A/IL-17F inhibition) resulted in rapid improvement, w/ resolution of PG lesions, marked reduction in HS drainage, and complete epithelialization of ulcers by 5 months. 👇🏻

✋ Clinical Case: Glomus Tumor as a Cause of Long-Term Pain

Glomus tumors are rare benign tumors of the fingers, well known for their ability to cause intense, debilitating pain while eluding diagnosis for years.

We present the case of a 62-year-old patient who had suffered from severe pain in her left index finger for 15 years. The pain was aggravated by cold and pressure—classic signs of a glomus tumor. An unusual feature was the pain radiating proximally, all the way to the shoulder, which misled physicians and led to prolonged misinterpretation of the symptoms.

🔍 Diagnostic Challenges
A previous MRI was inconclusive and revealed no pathological changes. However, the diagnosis was suspected clinically based on the characteristic history and triad of symptoms: localized pain, cold hypersensitivity, and tenderness to palpation.

✂️ Treatment
The patient underwent surgical removal of a subungual tumor measuring just 1 mm.

📌 Result
Immediately after the surgery, pain resolved completely and permanently. Histological examination confirmed the diagnosis of a glomus tumor.

🧠 Key Findings
▪️ Glomus tumors can present atypically, including radiating pain, which significantly complicates diagnosis.
▪️ Absence of findings on MRI does not exclude a glomus tumor.
▪️ In some cases, clinical judgment takes precedence over imaging.
▪️ Surgical removal is a radical and curative treatment.

Sporotrichoid Nodular Lymphangitis from Nocardia brasiliensis.

The nodules had first appeared on the back of her hand at the site of a thorn puncture she had sustained while gardening.

The lesions had spread up the hand along a red streak and then had appeared on the forearm.

An aspiration of the hand lesion was performed. Bacterial culture of the aspirate grew filamentous, gram-positive, acid-fast branching rods.
