Update: there's no history of renal insuficiency. The nodule is subpleural basal. She had uterine endometrioid carcinoma and thyroid papillary carcinoma. I found out she said something about a past lung surgery because of repeated pleural efffusion so maybe its related to that.
@yro854 Thank you so much! I confess I thought of kayexalate but I was thinking it could only be seen in a gastric biopsy (in the context of an aspiration it makes more sense). I will check her medical records to confirm!
@Dina_hussein94@JMGardnerMD I asked Prof Bostjan Luzar for help and he described the lesion and explained that it was necessary genetic testing to make the correct diagnosis although it has no convincing morphological criteria for melanoma. The patient didnt want to do the study...
83y male with a lesion on his nose (scar? desmoplastic melanoma?). There's a melanocytic nest in the dermis and inflammation and foreign body reaction but there is no record of previous excisions in this location. No mitosis. I really need some advise :) @JMGardnerMD

67 y/o male presented with epigastric pain for 4 mos and was found with a pancreatic head mass. EUS-guided FNA and bx show solid aggregates of epithelioid tumor cells with easily striped nuclei. Bcl-10 strongly pos. Dx: Acinar cell carcinoma.
