Based on the available references, it can be concluded that the risk of myocarditis is higher after COVID-19 infection compared to after COVID-19 vaccination. Reference Altman et al. (2023) states that previous reports have suggested that the risk of myocarditis during COVID-19 illness is estimated to be 100-fold greater than the risk of myocarditis from a COVID-19 mRNA vaccine. However, studies listed in Table S1 indicate a similar incidence between myocarditis occurring in COVID-19 infection and after mRNA vaccines. Reference Watanabe et al. (2023) supports this by stating that the risk of developing myocarditis after SARS-CoV-2 infection is higher than after COVID-19 mRNA vaccination. A previous nationwide study in Israel showed that the risk of developing myocarditis following SARS-CoV-2 infection was 18 times higher than the control group, while it was 3 times higher following vaccination than the control. Reference Stultz (2023) further confirms this by stating that adult analyses suggest a 7-fold higher risk of myocarditis due to COVID-19 infection compared with the vaccine. In the pediatric population, COVID-19 disease patients are associated with higher risk ratios for myocarditis/pericarditis or MIS-C compared to the risks after immunizations. Reference Frasca et al. (2023) provides additional evidence by reporting that the results of excess presentation of myocarditis cases reported following administration of the mRNA-1273 vaccine are high and exceed the frequency of these events after the first seven days following SARS-CoV-2 positivity. However, the risk after SARS-CoV-2 positivity tends to decline after 7 days post-positive test. Reference Davis et al. (2022) supports the conclusion by noting that the risk of extra myocarditis events in the month after COVID-19 vaccination is significantly lower compared to the risk of myocarditis after COVID-19 infection. Reference Liu & Kafil (2022) states that the risk of adverse cardiac events such as myocarditis/pericarditis after COVID-19 infection is still 1.8 to 5.6 times higher compared to after COVID-19 vaccination. Reference Urban et al. (2022) also supports this conclusion by stating that the risk of myocarditis after vaccination is lower than the risk of myocarditis after COVID-19 infection regardless of sex and age group. In summary, the available evidence suggests that the risk of myocarditis is higher after COVID-19 infection compared to after COVID-19 vaccination. However, it is important to note that the risk of myocarditis after vaccination is still present, although it is lower than the risk after infection. Further research and cohort studies are needed to fully understand the impact of vaccination on the incidence and severity of myocarditis after COVID-19 infection.
